2015-16 Returning Student-Athlete Paperwork Checklist
All of the following forms must be filled out completely and submitted to the sports medicine staff prior to any participation in intercollegiate athletics at Eckerd College.
If you have Asthma or ADHD and take medication:
The NCAA requires the following information be on file. It is your responsibilities to print the form and have your physician fill it out. Then return the forms to the Athletic Training Department.
Miscellaneous:
The NCAA requires you are informed about the following topics. A specific educational session will be arranged to cover these materials. You do not need to print the material but please read the Eckerd College Athletic Department drug policy and the information about concussion and sickle cell.
